- Insurance card and picture ID
- Referral (if necessary)
- Download and fill out Initial Visit Package forms (this will speed up registration process and decrease your waiting time)
- Part of this Initial Visit Package is how we use and disclose your personal information. Our policy is here for your review.
- List of ALL medications with their dose and frequency (included in Initial Visit Package forms)
- Pertinent medical records and test results
